Circle of Support

When putting together a Life Plan for your Special Needs loved one, you will want to make sure you include a comprehensive list of all the important people in your life, especially the people that will play an integral part of your loved one’s life if you are no longer around.

Your list should include family members, relatives, friends, co-workers, professional advisors, case workers, and any other community members.  Include anyone and everyone that is currently helping with the care of your loved one or that you think will be a big part of the plan when you are no longer available.  Remember to keep updated information on each person with details like addresses, phone numbers, relationship to individual, account numbers, and other pertinent information relating to your loved one.

Most importantly, you should have immediate access to all of this information.  In other words, you need to collect and organize all of the information about your Special Needs loved one and place it into one simple location.  We recommend you use a 3-ring binder to store all of this critical information.  This helps consolidate the details into one location and allows for updates whenever appropriate.  In addition, this will help future caregivers have the information they need, when they need it, should they have to step into your shoes.
