Life Plan Builder Step 2 – Personal Information

This section of your Life Binder helps you put all of your critical personal information into one easy-to-access location.  In addition, it helps you organize all the important people in your life and the life of your Special Needs loved one.  It combines all your pertinent information and places it at your fingertips when a future caregiver may need it most.  The information collected include specifics about you, your Special Needs loved one, your immediate family, your extended family, your important advisors, and your circle of support.

Please use the following forms to help you collect all of this critical personal information.

Use the checklist below to complete Step 2

  • Print the Step 2 Cover Sheet for your binder.
  • Complete the form Getting to Know You (select one option below)
    1. Option 1 – Open the Getting to Know You Adobe PDF version, print, and fill in the blanks with a pen.
    2. Option 2 – Open the Getting to Know You Microsoft Word DOC version, complete the appropriate sections, save it to your computer, and then print out a copy for your binder.
  • Complete the form Getting to Know Your Special Needs Loved One (select one option below)
    1. Option 1 – Open the Getting to Know Your Special Needs Individual Adobe PDF version, print, and fill in the blanks with a pen.
    2. Option 2 – Open the Getting to Know Your Special Needs Individual Microsoft Word DOC version, complete the appropriate sections, save it to your computer, and then print out a copy for your binder.
  • Complete the form Getting to Know Your Family and Circle of Support (select one option below)
    1. Option 1 – Open the Getting to Know Your Family and Circle of Support Adobe PDF version, print, and fill in the blanks with a pen.
    2. Option 2 – Open the Getting to Know Your Family and Circle of Support Microsoft Word DOC version, complete the appropriate sections, save it to your computer, and then print out a copy for your binder.
  • Complete the form Getting to Know Your Personal Advisors and Representatives (select one option below)
    1. Option 1 – Open the Getting to Know Your Professional Advisors Adobe PDF version, print, and fill in the blanks with a pen.
    2. Option 2 – Open the Getting to Know Your Professional Advisors Microsoft Word DOC version, complete the appropriate sections, save it to your computer, and then print out a copy for your binder.
  • Insert a completed copy of each one of the four forms into your binder behind Tab 2.

TeamworkYou should review and update your Personal Information from time to time (at least annually) to be certain that it is complete, accurate, and current.  In addition, you should take the time to personally talk to the important people and professional advisors in your life about your Life Binder.  It is a good idea to discuss it with them while you are still around.  Plus, this will further assist in building the relationship and will further enhance the success of your planning.

Although the templates provided above offer several sections to add important people in your life, it is quite possible that we have not included a section for someone important in your life.  Your loved one and your family are unique, so your list of VIP’s will be particular to your situation.  Feel free to edit and adjust the Word document to fit your specific needs and make it your own.
